Eligibility Requirements
- Enrolled in:
- Iowa Tribe of Kansas and Nebraska
- Sac and Fox Nation of Missouri in Kansas and Nebraska
- Must live in:
- Doniphan County
- Richardson County
- Brown County
- Jackson County
Prescription Requirements
- Already Have a Prescription?
- Must be valid and dated within the last 12 months
- Need a New Prescription?
Schedule eye exam with refraction
Referral Process:
• Must have been seen at WCHC in last 6 months
• If not, schedule at WCHC
• Referral goes through PRC process
How to Schedule
Once you have your prescription, call the clinic to book your optical time slot
Appointments available on Wednesdays
(Subject to change based on demand)
What’s Covered?
Adults: 1 pair/year, up to $500
Children: 2 pairs/year, up to $500
Contact Lens Users:
• Must follow same prescription process
• Contact the clinic to have an order submitted to McPeak Optometry in Hiawatha
